We are pleased you have chosen us as your team to guide you through this journey. Our goal is to provide you with the best possible care!   We invite you to take advantage of our many services and support offerings, as part of our comprehensive care facilities. To become a new patient complete the steps below:

1. Schedule an Appointment

You may become our patient either by making the appointment yourself or through a physician referral.  Please note that each provider of Ironwood Women’s Centers has his or her own scheduling number. To schedule an appointment, please call the provider of your choice.

2. Fill Out Your New Patient Forms

Please download and complete all of your new patient forms and bring them with you to your first appointment.

3. Prepare for Your Visit

Many patients find it helpful to write down their questions and concerns so they don’t forget to ask their provider.  You are welcome to bring someone with you to your appointment.
In addition to completing the forms, please bring the following on your first visit:

-Driver’s License or other valid government-issued photo identification.

-Health insurance cards.

-A list of all medications that you are currently taking including over-the-counter medications, remedies, vitamins, and herbal supplements.

4. Meet Your Team

At Ironwood Women’s Centers, you are never alone. We welcome you to learn about our team and treatment support services.

5. Create Your Patient Portal Account with CareSpace

Our patient portal is an easy way for you to communicate with your providers at Ironwood Women’s Centers.  At your first appointment, ask a staff member to verify your e-mail address so you can join our patient portal.  You will receive an e-mail with your login information within the day of your appointment.

Where does the information in CareSpace come from?

The information in CareSpace comes from your medical records at our practice.

Who can see my account?

Only you and the people you invite can see your account.  If you invite someone to your CareSpace account, they can see all of the information you can.

Is the information in CareSpace private and secure?

Yes, CareSpace is certified in the latest security standards, and information will stay private and secure.  CareSpace access is only permitted to authorized users who have been verified through a registration process.


Need Help?

Tips for registering your account:

My registration link has expired.  How do I set up my account?

To make sure that your information stays safe, registration links expire after four days.  Give our practice a call if you need to be sent a new link.

What do I do if I never received a registration link from my practice?

Check your spam folder.  If you still don’t see one, give our practice a call.

Where do I log in for CareSpace?

You can always access CareSpace by visiting www.ironwoodcrc.com/portal from the browsers on your personal computer, tablet, or mobile device.

What happens if I forget my password?

No problem, you can reset your password yourself.  Look for the “forgot password” link on the www.ironwoodcrc.com/portal  page.
